Your First Visit
Patient Forms
Upon entering our office, our receptionist will greet you and welcome you as a member of our team. We recommend that you fill our our Patient Intake Form (click for additional forms) prior to your appointment to save time when you arrive at our office. This paperwork provides us with general information about yourself and your condition. |

Consultation Next, you will have
a consultation with either Dr. Daphne or Dr. Brent to discuss your
health-related problems, concerns, as well as potential treatment
options. Your first visit is designed for the doctor to learn more
about you, your condition, and your expectations to determine
wether or not chiropractic will meet your goals |

Examination Following the consultation, Dr. Brent or Dr. Daphne will perform a complete chiropractic exam that involves postural analysis, specific orthopedic and neurologic testing, functional analysis, range of motion, surface electromyography, and static/motion palpation of your spine. Nothing will be done in our office without your consent. |

X-ray
Studies Your
specific case may require us to take x-rays for a full evaluation
to help us determine the root cause of the problem you are seeking
care for. We also use X-rays to rule out more serious conditions
and assists us in developing the most effective treatment plan for
you. |

Doctor's Report Once all the information is gathered and examination performed, One of the doctors will give you a full report of findings to answer the most common questions asked before initiating care: - Can you help me?
- What do you recommend that I do to get better?
- How long will it take?
- How much does it cost?
- What kind of rehab/execises will I be doing?
The doctor will discuss his/her recommendations with you following addressing your health history and goals, examining your spine, and reviewing any x-rays or tests. If you have a condition that requires care with other providers, the doctor wil tell you. If the doctor believes he/she can help you, we will recommend treatment and a wellness care program. |
